What You'll Do

As a BOSS Transportation Mentor, you'll help families overcome transportation barriers that impact their health, employment, education, childcare, and overall stability. Through mentoring, case management, resource navigation, and direct transportation support, you'll empower participants to move toward greater self-sufficiency. In this role, you will:

  • Provide intensive case management, mentoring, and support to families enrolled in the BOSS program.
  • Assess transportation needs and identify barriers affecting participant goals and stability.
  • Develop and monitor individualized action plans that support family success.
  • Provide transportation for approved activities related to healthcare, employment, education, childcare, and other self-sufficiency goals.
  • Connect participants with community resources, healthcare services, transportation providers, and public benefit programs.
  • Maintain regular contact with participants through phone calls, meetings, community visits, and occasional home visits.
  • Document services, transportation supports, participant progress, and outcomes in agency systems.
  • Build strong relationships with community partners and collaborate to address participant needs.
  • Complete required training, including Community Health Worker (CHW) certification, and participate in ongoing professional development.
  • Ensure compliance with program requirements, confidentiality standards, and safe transportation practices.

Most importantly, you'll serve as a trusted mentor and advocate, helping families overcome barriers, access critical resources, and build a path toward long-term stability and self-sufficiency. This is a hybrid position, with remote work from a home office and extensive travel throughout the service area.

What We'll Love About You:

  • Associate Degree in Social Work, Human Services, Sociology, Psychology, Public Health, Community Health, or a related field required. Bachelor's Degree preferred.
  • Two years of experience working with vulnerable populations, families experiencing poverty, case management, mentoring, transportation coordination, community health navigation, or a related human services field required.
  • Experience with participant goal planning, resource navigation, community partnerships, and case documentation preferred.
  • Strong communication, organizational, and relationship-building skills, with the ability to work independently and as part of a team.
  • Proficiency with Microsoft Office, databases, electronic documentation systems, and participant tracking software.
  • Valid Michigan driver's license and reliable transportation required.

Most importantly, you're passionate about helping families overcome barriers, build self-sufficiency, and achieve their goals through compassionate support, mentoring, and advocacy.
