**Adult Outpatient Case Manager**

The Adult Outpatient Case Manager responds to and develops initial

crisis management and recovery plans for clients. The ideal candidate

will embrace excellent customer service with resource information and

connecting clients to immediate access of services reducing the barriers

to treatment. In this dynamic role the Adult Outpatient Case Manager

will also regularly provide early screening and intervention; case

coordination; urgent next day follow-up and triage appointments.

**RESPONSIBILITIES & SCOPE**

• Provides immediate response upon referral receipt to provide:
  
  

excellent client service; resource information; expeditious access

to services; identify and reduce barriers; increase problem solving;

develop initial treatment plan goals; early screening and

intervention; case management; urgent next day follow-up and triage

appointments.

• Understanding of community and agency resources and the ability to
  
  

make appropriate referrals.

• Conduct early screening, intervention, and crisis counseling,
  
  

including risk assessment and triaging immediate needs.

• Provide comprehensive case management, coordination, and referral
  
  

services.

• Maintain accurate clinical records and documentation.
• Actively participate in clinical team meetings to inform and
  
  

coordinate services.

• Consultation and educational services to the community, area
  
  

agencies, professionals, and schools as required.
