DS Resource Coordinator
Full-Time | St. Johnsbury & Derby
The DS Resource Coordinator plays a vital role in supporting individuals in the Developmental Services (DS) and Adult Family Care (AFC) programs by developing housing resources and recruiting Shared Living Providers. This position works collaboratively with individuals, families, providers, agency staff, and community partners to ensure safe, supportive, and person-centered living environments.
Primary Responsibilities include, but are not limited to:
- Recruit, interview, and support Shared Living and Adult Family Care home providers
- Conduct home studies and complete required documentation for provider contracts
- Collaborate with individuals, families, guardians, and interdisciplinary teams to identify housing and resource needs
- Provide housing support services tailored to the unique needs of individuals served
- Maintain accurate electronic medical and program records in compliance with State and Federal guidelines
- Collaborate with DAIL partners and maintain the DAIL housing portal
- Facilitate and participate in team meetings and transition planning
- Coordinate and facilitate trainings for contracted home providers
- Support home providers as individual needs evolve
- Work independently to meet program requirements
Position Qualifications:
- Bachelor’s degree in Human Services or a related field
- 1–2 years of experience working in Developmental Services programs (or equivalent experience considered)
- Knowledge of DS and AFC waiver-funded programs, or willingness to learn
- Strong communication, organization, and collaboration skills
- Valid driver’s license, reliable vehicle, and required auto insurance
- Ability to pass all background checks

- Pay $21.25 to 25.97 Based on experience
