Care Coordinator

Our mission is to empower individuals, families, and communities by promoting hope, healing, and support.  NKHS is a Designated Agency contracted with the State of Vermont to provide the highest quality of service in the areas of adults with intellectual/developmental disabilities, children and youth with serious emotional disabilities, and adults with mental health and substance use challenges.  We serve the populations of Caledonia, Essex, and Orleans counties through a wide variety of targeted programs for individuals of all ages. RESPONSIBILITIES & SCOPE

• Promote community integration of individuals and assist/encourage natural supports necessary.
• Counsel clients about the use of the National Suicide and Crisis Lifeline, 988, local hotlines, warm lines, mobile crisis, and other needed resources.
• Assess clients need/desire in accessing benefits, such as Medicaid and financial assistance.
• Develop partnerships with a variety of community services and supports.
• Contact any clients that have been discharged from an inpatient setting within 24 hours of discharge to connect them to services.
• Provide service coordination with a medical focus among internal and external providers.
• Reviews schedule daily and contacts clients that no show and reschedule immediately. Discusses barriers to attending appointments.

QUALIFICATIONS

• High school diploma or equivalent.
• At least 2 years’ experience in a behavioral health or primary care office setting

• Competitive Salary: $19.50 - $23.84 per hour, based on education and experience.
