Job Overview
We are seeking a dedicated and compassionate Direct Support Professional and Home Health Aide (HHA) to join our team. The ideal candidate will possess a strong commitment to patient-centered care, with experience working with diverse populations including seniors, individuals with developmental disabilities, and those requiring memory or hospice care. This role offers an opportunity to make a meaningful difference by supporting clients with their daily activities and medical needs in a respectful and dignified manner.
Duties
- Provide personal care assistance including bathing, dressing, grooming, and mobility support using patient transfer methods such as Hoyer lifts when necessary.
- Assist clients with Activities of Daily Living (ADLs) such as meal preparation, light housekeeping, laundry, and medication reminders while maintaining a clean and safe environment.
- Monitor patient health by observing vital signs, reporting changes, and documenting medical information accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Support clients with specialized needs including Alzheimer's disease, dementia, memory care, and end-of-life care, ensuring comfort and dignity at all times.
- Administer medications following home healthcare regulations and ensure compliance with HIPAA standards to maintain patient confidentiality.
- Communicate effectively with clients and their families to provide updates on health status and address concerns promptly.
- Follow protocols for emergency response including first aid, CPR, basic life support (BLS), and hospice care procedures when necessary.
Qualifications
- Prior experience as a Home Health Aide or in assisted living, nursing home, or long-term care environments is highly preferred.
- Knowledge of medical terminology, catheter care, medication administration, and vital signs monitoring.
- Experience working with individuals with developmental disabilities, geriatrics, dementia, or memory care needs.
- Familiarity with home health care assistance regulations and documentation practices using EMR/EHR systems.
- Certification in first aid and CPR is required; additional training in hospice or end-of-life care is advantageous.
- Ability to lift heavy objects safely and perform physical tasks associated with patient transfer methods such as Hoyer lifts.
- Strong communication skills coupled with compassion for providing patient personal care assistance in a respectful manner.
