ORGANIZATIONAL BACKGROUND
POSITION SUMMARY
The Director of Care Coordination provides strategic leadership and operational oversight of Northeast Health Partners’ (NHP) care coordination program. This position is responsible for ensuring the delivery of high-quality, person-centered, and integrated care coordination services for members with complex physical health, behavioral health, substance use disorder, and social needs.
The Director leads the development, implementation, and continuous improvement of care coordination strategies to ensure services meet member’s needs, achieve measurable outcomes, and comply with applicable state, federal, and contractual requirements. The position provides leadership and direction to care coordination staff, including oversight of staffing, supervision, training, performance management, workload distribution, and program operations.
The Director is responsible for ensuring timely and effective identification, outreach, assessment, care planning, care transitions, and coordination of services for members, with particular attention to individuals with complex or high-risk needs. The position promotes a whole-person approach that integrates physical health, behavioral health, substance use disorder services, and health-related social needs.
The Director works collaboratively with NHP leadership and departments, providers, hospitals, community organizations, county agencies, schools, state partners, and other stakeholders to strengthen care integration, address barriers to services, and improve member outcomes.
The Director monitors program performance, utilization, quality measures, member outcomes, and contractual deliverables and uses data to identify opportunities for improvement. This position ensures appropriate documentation, reporting, audit readiness, and adherence to NHP policies and regulatory requirements.
The Director serves as a key organizational leader responsible for advancing health equity, strengthening member engagement, supporting effective transitions of care, and ensuring NHP's care coordination program delivers meaningful and measurable value to the members and communities it serves.
DUTIES PERFORMED
Oversee the design, implementation and operational management of all Care Coordination requirements as outlined in ACC 3.0 Contract.
Facilitate effective communication, collaboration, and coordination within and between NHP and partner PCMPs, CMAs, BHASOs, and D-SNPs with the goal of ensuring comprehensive, effective and efficient coordination of care for jointly managed Members.
Build and nurture trusted and cooperative working relationships with all partner PCMPs, CMAs, BHASOs, and D-SNPs.
Manage Care Coordination processes and staff, providing oversight, monitoring, and training as required.
Oversee effective delivery of Care Coordination for Members by Network Providers, delegated and Subcontracted organizations, and others within the Health Neighborhood.
Create proactive processes and policies to prevent the exacerbation of Member conditions and the escalation of Member situations.
As a member of the Executive Management team, work cooperatively with others to develop and support the vision, mission and values of the agency. Ensure others foster agency vision, mission and values through demonstrated actions and communications within and outside the agency.
Provide leadership, motivation and direction to the community partners and ASO, particularly for the programs and initiatives assigned, and to the agency as a whole.
Accountable for the development, implementation, and functions of assigned program(s) and is responsible for ongoing administration. May be responsible for the development of agency wide initiatives.
Responsible for policy and procedure development and participates in overall agency policy and procedure development and implementation.
Collaborate with other Executive Management team members to ensure the development of training and oversight strategies assuring teams and agency functions are in compliance with all necessary regulations and agency policies and are in the best interest of the members served. Participate in the development of region-wide strategies to ensure same.
Ensure position presents with an accurate and favorable representation of the agency.
Provide required reporting and documentation/deliverable expectations as required by contract.
May provide oversight and/or primary responsibility for non-clinical programs essential to the functioning of the agency (e.g. Quality Improvement/Assurance, Compliance, Care Coordination). Ensure non-clinical programs are complementary and collaborative with the Executive Management team, program managers, and other functions of the agency.
May be called upon to take the lead in special projects that are critical to agency success. In this role as project director, the Director of Care Coordination will work closely with agency executives and middle management personnel to develop, implement, train and evaluate success.
Develop and maintain cooperative, collaborative and effective relationships with community partners that enhance the reputation of the agency. Maintain availability to facilitate presentations to various community groups and organizations to increase understanding and improve the image of Northeast Health Partners in the community/region.
Ensure fiscal viability of Northeast Health Partners; pursue and maintain funding sources and actively engage in budget development and management utilizing necessary applications and/or reports.
Perform other duties as assigned.
Ability to attend functions, including boards, community events, and NHP sponsored events during non-routine work hours including evenings and weekends.
Ability to routinely travel throughout Colorado and Region 2.
Salary Range: $90,000–$120,000 annually, commensurate with qualifications and experience.
KNOWLEDGE/SKILLS/ABILITIES
Education/Credentials/Experience:
REQUIRED
Be located in Colorado.
Registered Nurse, master’s level social worker or equivalent health care professional with necessary clinical experience.
5 years’ experience in an upper level management position, plus
3 years’ experience in the care coordination and/or program areas to which the position is assigned within the past ten years.
Knowledge of Medicare and coordinating care for individuals with disabilities.
Experience leading initiatives in the assessment of programs and processes, development of solutions, and implementation.
Advanced proficiency with software products (e.g., Microsoft Office Suite, Visio, and/or statistical software like SAS or SPSS), data entry, and data management.
Extensive understanding of rules, regulations and laws governing behavioral health practice and experience in practical application at the program and agency level.
PREFERRED
Experience in the development and management of in-service training and education.
Understanding of data and reporting.
Management and fiduciary experience in a healthcare or nonprofit agency.
Preference given to applicants residing in and familiar with the contract region.
Knowledge of:
Mild to severe mental health and/or substance use disorders/disabilities, trauma-informed care, and recovery/resiliency-oriented services.
Member groups and/or issues related to the program area (i.e. age groups, level of severity, risk category, etc.).
Best practices for program development and enhancement, change management, and effective supervisory/management approaches.
Compliance with 42 CFR Part 2 in addition to confidentiality and HIPAA rules.
Professional and ethical guidelines as outlined by the State of Colorado.
Federal, state and local regulations regarding delivery and documentation of behavioral health services.
Report writing and review. Ability to gather and synthesize information and contractual requirements and present to State officials.
Electronic health record documentation practices.

WORKING CONDITIONS
Work is generally performed in a standard office environment, with high usage of video and teleconferencing technology; may perform duties from remote environment as approved. Periodic travel within the state/region and occasional overnight travel may be required.
