Position: Senior & Wellness Program Coordinator (SWPC)
Reports to: Senior & Wellness Programs Manager- Jennifer Smidt
Description: The SWPC will provide referral trainings, specifically to the Healthcare systems, FQHC’s, free clinics and Primary Care settings. Also provides system change and referral resources and activities to providers throughout our seven counties. This position identifies providers that have decreased in the referral process and engages in different stages of communication to assist them in increasing their referrals. This position also focuses on some admin duties as well as facilitating senior and wellness program.

Specific Tasks:
• Conduct presentations/trainings with our community partners with the goal of increasing referrals and expanding our reach.
• Conduct System Change activities with our partners such as trainings, Lunch & Learns, etc.
• Work with the Senior Program Manager and Executive Director to develop, execute, and evaluate partnerships with community organizations, health systems, local free clinics, FQHC’s within the senior program.
• Implement Northeast Florida AHEC’s marketing plan according to guidelines.
• Collection and input of all referrals and update monthly reports in database.
• Compile and complete appropriate data for Senior Program reports.
• Implement and manage facilitator agreements.
• Continue positive relationships with current partners while developing additional relationships to further Senior programming efforts.
• Facilitate senior programs
• Schedule and facilitate monthly referral meetings with Senior Program team.
• Analyze referral data and provide updates and feedback to the team as well as next steps.
• Data-entry
• Prepare Targeting & Outreach report.
• Assist with collecting program paperwork.
• Assist with scheduling programs.
• Assist with preparation of paperwork for audits and program monitoring.
• Register participants for scheduled programs.
• Assist with assembling program kits.
• Assist with Health/Wellness Fairs, Lunch and Learns, Presentations.
• Assist as needed, with follow-up call process to participants that have attended classes.
• Provide system change activities and other support to specific provides (FQHC, Community Clinics, Health Department, etc.) that will help grow referrals.
• Prepare quarterly referral letters to providers.
• Prepare and distribute referral outcome letters.
• Implement and maintain the Provider Plaque system with providers to include presenting and reporting.
• Work with Senior Program Manager on building and establishing relationships with community partners.
Qualifications:
• The preferred candidate will have a four-year degree in a health-related field or equivalent work experience.
• Coordination and networking with community partners
• Understanding of all populations across the seven-county catchment area
• The candidate must possess excellent organizational and people skills.
• Previous experience in presenting to groups of people, as well as proficiency in word processing and database management.
• Knowledge and/or experience with marketing principles and implementation are essential.
