What are the responsibilities and job description for the Case Manager- Forensic Assertive Community Treatment Team position at Northeast Counseling Services?
The FACT Team is a comprehensive community-based treatment for individuals with severe mental illness and are involved with the legal/criminal justice system. The Case Manager provides case management activities through an aggressive Forensic Assertive Community Treatment Team outreach approach in the consumer's community.
The Forensic Case Manager intervenes with individuals who have been diagnosed with a mental illness and are involved in the criminal justice system. The primary goal is to assist these individuals with establishing more productive lives. This workforce member has access to the complete chart for cases requiring workers involvement.
QUALIFICATIONS/EDUCATION: The position requires a bachelor's degree (Masters preferred) in social work, psychology, counseling or other related field from an accredited college or university; a bachelor's degree in a non-clinical field plus two years' experience. Valid Pennsylvania Driver's License is required.

Schedule:
- 8 hour shift
- Alternating shifts: Day shift (8:30am-5:00pm), Second Shift (12:00pm-830pm) weekends, holidays
- Monday to Friday and occasional weekends
