Northeast Arc, Clinical Services is seeking a Clinical Nurse Case Manager to oversee and support our dedicated clinical team. This is a dynamic role for a skilled clinician who thrives in a collaborative environment and is committed to delivering exceptional patient outcomes at home.
What You’ll Do As a Clinical Manager
- Lead and mentor a team of nurses, ensuring high-quality care and compliance with regulations.
- Collaborate with the Clinical Director and fellow managers to optimize staffing and patient care.
- Build and maintain strong relationships with referral sources, families, and physicians.
- Oversee clinical documentation, care plans, and assessments (OASIS, 485s, etc.).
- Participate in QAPI initiatives and staff education.
- Provide occasional on-call support and respond to incidents and concerns with professionalism and empathy.
- Licensed Registered Nurse (RN) in Massachusetts with 3 years of nursing experience, familiar with Continuous Skill Nursing (private duty nursing).
- Proven leadership or case management experience.
- Strong clinical judgment and communication skills.
- Proficiency with electronic health records and software applications.
- Valid driver’s license and ability to travel locally.
