Title: Program Case Manager
Schedule: Monday-Friday 8am-4pm
Job Summary: Provides supervision, opportunities, and skill development to participants served in NeArc�s community-based day and work support program. Provides supervision, guidance, and training to assigned program case managers.

Pay Rate: $25 per hour.

Major Responsibilities
- Evaluate individuals progress periodically and make adjustments as needed to improve outcomes
- Evaluate and determine program participants� interests and develop individual skills.
- Provides assigned caseload oversight by attending scheduled meetings, maintaining regular and professional contact with the participant�s team, completing all required documentation, and ensuring the participant�s progress in the program as determined by his or her goals.
- Creates individual goals to reflect the participant�s vision regarding skill development.
- Maintains all required documentation within required timeframes to include:
Monthly/Quarterly Progress Notes/6-month summaries in HCSIS
Assessments
Confidential Record updates
- Provides transportation to participants served in a personal or agency vehicle when one is available.
- Assists in obtaining appropriate volunteer placements for participants served and provides ongoing coaching support in these placements to ensure continued success.
- Attends and completes all required training assigned by the supervisor.
- High School Diploma or equivalent required. 1 year of experience working with intellectually and/or developmentally disabled adults or other related experiences preferred.
- Excellent organizational and interpersonal skills. Effective communicator in both person and writing. Ability to communicate in English and or ASL via direct communication.
- Valid driver�s license and reliable transportation is required.
- Must be able to lift to 50 lbs. as needed.
- Must be able to stand/walk up to one hour uninterrupted.
Pay Rate: $25 per hour.
