At NorthBay Health the Clinical Coordinator, Hospital Hemodialysis, is a Registered Nurse with 24-hour responsibility and accountability for the clinical operations of the dialysis program, including hemodialysis (HD) and continuous renal replacement therapy (CRRT). This role provides frontline leadership, overseeing daily operations and ensuring the safe, effective delivery of patient care. The Clinical Coordinator works collaboratively with the Clinical Nurse Manager, and under the supervision of, the Clinical Nurse Director of Critical Care Services.

The Clinical Coordinator also serves as a liaison with contracted vendors providing services including, but not limited to, plasmapheresis, apheresis, and peritoneal dialysis. Responsibilities include fostering effective working relationships with physicians and ensuring staff adherence to hospital policies, procedures, and guidelines. The Clinical Coordinator manages human and financial resources appropriately and may delegate patient care responsibilities to qualified healthcare personnel in accordance with their scope of practice and job descriptions.

The Clinical Coordinator, Hospital Hemodialysis, has the following responsibilities, including but not limited to:

  • Managing the triage, coordination, and prioritization of hemodialysis (HD) and continuous renal replacement therapy (CRRT) treatments for patients.
  • Assisting in the coordination of care across departments.
  • Assigning staff duties and designating responsibilities.
  • Serving as a clinical resource, competent in all dialysis modalities.
  • Evaluating staff performance, including quality of work, productivity, and professionalism of the dialysis team.
  • Addressing and resolving employee, physician, and patient concerns during the shift.
  • Ensuring quality assurance for equipment safety, processing, regulatory compliance, and reporting.
  • Collaborating with the Critical Care Nurse Specialist (CNS) and Nursing Education Specialist (NES) for staff education and competency validation.
  • Applying Jean Watson’s Theory of Caring, Relationship-Based Care, and NorthBay’s Professional Nursing Practice Model in patient care.
  • Supporting the organization’s mission, vision, values, and standards of excellence.
  • Assessing, planning, implementing, and evaluating care for all assigned patients.
  • Monitoring patient progress toward goals and maintaining therapeutic relationships with patients, families, and the interdisciplinary team to promote, maintain, and restore health.
  • Exercising responsibility, accountability, and authority to deliver safe, effective, high-quality nursing care in alignment with the goals and philosophy of the Patient Care Services Division.
  • Ensuring care meets the physiologic, developmental, and psychosocial needs of patients.
  • Providing leadership and supervision of direct care staff and overseeing daily dialysis operations.
  • Contributing to quality nursing care through performance improvement initiatives that support positive patient outcomes.
  • Performing other duties as assigned.

Qualifications:

  1. Education:  Graduate from an accredited school of professional registered nursing.  Bachelor's degree or higher in Nursing required (Employees who are grandfathered are exempted).

     

  2. Licensure: Valid and current RN license by the State of California.  Current AHA or AHA equivalent BLS and ACLS.

     

  3. Experience:  Minimum of 5 years’ clinical experience in nursing practice of dialysis. Minimum 1-year supervisory experience preferred. Demonstrated leadership abilities. 

     

  4. Communication:  Proficient written, verbal, and electronic communication skills in the English language.

  5. Standards of Performance: Demonstrate performance by adhering to established policies and procedures and exhibiting the defined characteristics associated with attendance and punctuality.  Attendance is an essential function of the job.

  6. Physical Skills:  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  7. Physical Effort:  Performs all physical duties of the job.  Requires lifting, bending, pushing, pulling, sitting, standing, walking, kneeling, twisting, reaching, and fine motor skills. 

  8. Hours of Work:  Variable

  9. Other: Completion of annual organizational requirements for employment.

  10. Department Specific: Per job description addendum as defined by each unit.

  11. Compensation: $88.83 - $107. (Max rate is commensurate of 20 years of experience in role)
