Employment Type: Full-Time (32 hours/week, 8:00am–4:30pm with weekend/holiday rotation)

Position Summary
The MDS Coordinator is responsible for the accuracy, completion, and submission of Minimum Data Set (MDS) assessments in accordance with state and federal guidelines. This role ensures resident care plans are individualized and updated, coordinates interdisciplinary care conferences, and plays a key part in quality assurance and compliance initiatives.
Key Responsibilities
Coordinate and complete MDS, Care Area Assessments (CAAs), and related documentation per CMS guidelines.
Ensure accuracy of MDS data and timely electronic submission through QIES/iQIES.
Lead development and updates of individualized, resident-centered care plans.
Schedule and facilitate interdisciplinary care conferences; ensure resident and family involvement.
Collaborate with staff to ensure care plans are implemented and monitored effectively.
Participate in QAPI activities, audits, and tracking of quality measures.
Educate staff on MDS documentation, care planning, and compliance requirements.
Support the nursing team, including participation in on-call, weekend, and holiday rotations.
Qualifications
Associate’s degree in Nursing required; Bachelor’s degree preferred.
Current RN license in Minnesota (required).
CPR certification (required).
MDS certification (required within 6 months of hire).
Previous MDS and nursing leadership experience, preferably in long-term care.
Strong communication, organizational, and leadership skills.
Proficiency with Microsoft Office Suite.
Physical Requirements
Ability to sit, stand, and move for prolonged periods.
Must be able to lift up to 50 pounds as needed.
