Our Mission: We are professionals committed to serving others, nurturing life, and promoting an individual’s ability to thrive.Position Summary:The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director ofSocial Services and/or Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basisResponsibilities:Assisting Director of Social Services Making phone calls to set up transportation or doctor appointments for residentsTransporting residents to doctors' appointmentsMust be able to assist social worker(s) in supporting patients and families through education, financial planning assistance, liaison with community agencies, etc.Providing residents and family Medicare/Medicaid and financial information when requestedMaintaining accurate social service progress notesUpdating the social services directoryObtaining information from community social health and welfare agencies in order to meet the resident’s needsPlanning and organizing time and duties to efficiently maintain caseloadProtecting each resident’s file and data with confidentially per HIPAA standardsMust perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitationQualifications:Must be a high school graduate or equivalentPrevious experience in skilled nursing preferredMust be able to read, write, speak, and understand the English languageStrong communication skillsAbility to multitask in a busy settingCNA certification preferredValid California Driver's license that is in good standingIf you meet the qualifications above, please apply now.
