Gardens of North Olmsted is seeking a compassionate and motivated Physical Therapist (PT) to join our in-house rehabilitation team on a PRN basis. This is an excellent opportunity for a therapist seeking flexible hours while making a meaningful impact in a skilled nursing and long-term care setting.

Responsibilities
Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services in accordance with physician orders and established plans of care
Assess mobility, strength, balance, and functional performance
Monitor resident progress and modify treatment plans as appropriate
Collaborate with nursing staff and interdisciplinary team members
Complete accurate and timely documentation using Net Health and PCC
Educate residents and families regarding treatment goals and recommendations
Qualifications
Graduate of an accredited Physical Therapy program
Current Ohio Physical Therapist license
SNF/LTC experience preferred but not required
Strong communication and clinical skills
Commitment to providing compassionate, high-quality resident care
