Administrative
- Plans, directs, coordinates, and evaluates Social Work and Activities Programming in accordance with NMHS Policy/Procedures and criteria established by various legal/regulatory accrediting bodies
- Manages the overall operations of the Social Work Department
- Supervises, oversees, schedules, and coordinates the activities of the Activities Coordinators and Activities Assistants on all shifts
- Assures all Social Work staff are properly trained regarding regulatory guidelines and daily work.
- Coordinates employee training.
- Provides psychosocial assessment of factors related to the resident’s illness and need for care
- Identify and provide problem resolution associated with high risk indicators endangering resident’s mental and physical health
- Identify support system
- Evaluate resident(s) and implement treatment plan
- Consults with residents as needed including provision of counseling and direct social worker services to assist with meeting psychological needs of resident/family
- Ensures residents/families receive necessary continuity of care and discharge planning needs.
- Act as a liaison between resident/family and treatment team
- Provides discharge/transition options to resident(s)/family(ies)
- Document assessment(s), progress, notes, etc. in resident’s medical record(s)
- Identifies and utilizes resources to best meet resident/family needs including interaction with payers and external stakeholders
- Develop and maintain effective working relationship with resources providers
- Researches, develops and maintains community involvement in social work services
- Ensure effective implementation of admission and discharge planning, provide new resident/family admission orientation.
- Communicates goal progression and plan of care to residents, staff and physician. Communicates agency mission and services to potential referral sources.
- Actively participate in volunteer recruitment and retention
- Provide educational programs to staff, residents, families, and the community
- Attends educational classes/programs to stay abreast of the changes in healthcare
- Complies with regulator and accrediting agency requirements
- Adheres with NMMC/NMHS policy/procedures and guidelines
