Position Summary
The Licensed Practical Nurse/Registered Nurse provides nursing care in accordance with standard nursing practice and in accordance with North Hill standards of care. The Licensed Practical Nurse/Registered Nurse will assist in conducting clinical rounds and collaborate with the clinical team and the Care Partners regarding Resident care needs.

Position Summary

Job Specific Duties

- Assists in administering medications per Vista Terrace policies, procedures, and state regulations
- Ensures medication documentation is accurate and complete
- Responds to Resident requests for support and uses communication systems appropriately
- Monitors and collaborates with Care Partners and coaches them to ensure Resident care meets North Hill standards
- Responds to Resident requests for assistance and care
- Answers personal emergency response system (PERS) calls promptly
- Actively participates in household routines
- Ensures Resident care needs are met by communicating effectively with the management team and by answering resident requests for care and PERS alarms in a timely manner
- Ensures all communication is carried over from shift to shift
- Updates individual Resident care plans in collaboration with the Resident, responsible party, and interdisciplinary team
- Recognizes and reports all allegations of Resident abuse, neglect, exploitation, social media exposure, and/or misappropriation of Resident property to the Director of Vista Terrace, the Wellness Nurse Managers, or the Nursing Supervisor and supports the investigation as needed
- Ensures physician order are transcribed accurately and executed
- Participates in QA program
- Ensures Pharmacy and treatment supply usage is efficient to ensure costs to Residents are accurate and not wasteful
- Completes incident reports on a timely basis. Notifies family or point of contact according to policies and procedures
- Reports hazardous conditions, equipment, and incidents to the supervisor on the shift they occur
- Follows safety requirements to include Emergency Management Plan and established Infection Control and Universal Precautions policies and procedures
- Follows safe food handling per Serve Safe guidelines
- Supervise Care Partners and ensure completion of Resident Care duties
- Meets department uniform, appearance, and grooming requirements
- Complies with all OSHA safety regulations, applicable local laws regarding health, safety, or environment

Required Skills And Abilities
- Ability to maintain confidentiality and strong customer support skills
- Ability to work independently with limited supervision
- Must possess the ability to deal tactfully with Team Members, Residents, family members, visitors, and the general public
- Work professionally with people from a wide variety of cultural and socio-economic backgrounds
- Complies with protection of health information (HIPAA) safeguards
- When working in Resident areas, must be able to communicate effectively in English
- Attends and participates in in-service educational classes, memory care training, Healthcare Academy and staff meetings as directed
- A current license as a Licensed Practice Nurse or Registered Nurse in the state of Massachusetts
- Current CPR and AED Certification
- Minimum of 2 years nursing experience in a hospital, assisted living, long term care facility or other related health care facility is required
- Experience working with dementia patients is preferred
- Proficient with computer software programs and EMR
