Key Responsibilities:
- Patient Care Management: Deliver direct, in-person nursing care including assessments and symptom management.
- Documentation: Maintain accurate documentation of all patient interactions and care in line with organizational policies and procedures.
- Admission Support: Perform admissions as needed (dependent on caseload)
- After-hours Coverage: Provide occasional assistance with on-call coverage.
Position Requirements:
- Current Hawaii RN license
- Preferred experience in hospice or home health
- Strong collaboration and communication skills for effective teamwork with the hospice interdisciplinary team
- Ability to provide patient education and counseling
- Valid driver’s license and reliable, insured transportation
- Strong clinical assessment and critical thinking skills
- Ability to work independently and manage time effectively
- Must be able to travel to the areas we serve: Hamakua, North Kohala, Waimea, and Waikoloa
