What are the responsibilities and job description for the Case Aide position at North Florida Office of Public Guardian, Inc.?
* Assist Guardian Representatives with scheduling medical appointments for
wards, accompany wards to medical appointments and visit wards that are in the
hospital.
* Deliver goods to wards and make deliveries to Group Homes or Facilities.
* Input data in Office of Public Guardian systems, I.e. Docubase or Wellsky.
* Review Medicare/Medicaid eligibility and resolve issues as assigned by the
Guardian Representative.
* Prepare and process case sensitive forms, applications and other paperwork as
need or assigned by Guardian Representative of Director of Case Management.
* Maintain on going communication /collaboration with Guardian Representative
regarding ward cases.
* Responsible for attending all regularly scheduled administrative staff meetings.
* Document all activities performed on wards behalf in the ward management
database as assigned by the Guardian Representative or Director of Case
Management.
* Assist the Guardian Representative in maintaining good documentation of
activities in EMS.
* Assist the Guardian Representative with accessing and monitoring care provision
to wards in the areas of residential and social.
* Assist the Guardian Representative with coordinating ward's medical care and
mental social services.
* Assist the Guardian Representative with preparing annual reports of persons on
all Office of Public Guardian wards.
* Assist the Guardian Representative with managing applications and
redetermination process of public benefits.
* Assist the Guardian Representative marshal assets and help the Guardian
Representative with the completion of all inventory form, i.e. Initial, Annual and
Finals.
Pay: $20.00 - $25.00 per hour
