Supported Living Nurse

In this role, you will work directly with individuals in their homes and communities to help them navigate daily life and reach their personal goals.

Duties

• Daily Living Skills: Providing prompts and assistance with basic grooming, such as brushing teeth, hair care, and dressing.
• Household Support: Helping individuals maintain a clean living environment, including room organization and laundry tasks.
• Social & Communication Skills: Supporting individuals in learning appropriate social interactions and building communication skills for community and workplace success.
• Community Integration: Facilitating involvement in community activities, such as visits to the library, local parks, or recreational centers.
• Goal Implementation: Working on specific objectives outlined in the individual's support strategy to foster independence.

Requirements/Qualifications

• Reliable Personal Vehicle: Must have a dependable vehicle to transport individuals to various community activities, appointments, and outings.
• Valid Documentation: Must possess a current driver’s license, valid vehicle registration, and current vehicle insurance.
• Insurance Minimums: Bodily injury limits must meet NES minimum requirements (typically $100,000 per person and $300,000 per occurrence).
• Commitment to Mission: A genuine desire to help individuals achieve a high quality of life and greater independence.
• Background & Safety: Ability to pass a mandatory background check and drug screening.
• Professionalism: High level of integrity, reliability, and the ability to maintain documentation accurately.
