We're looking for a caring, community-minded Family Support Specialist to join our Healthy Families team as a home visitor in Jefferson County!
About Us:
NCPPC is a nonprofit dedicated to improving maternal, child, and family health outcomes across the North Country region.
Healthy Families Jefferson is an evidence-based, accredited parent support and early intervention program that focuses on strengthening families, promoting positive parent-child bonding, and fostering child health and development. We follow the Healthy Families America (HFA) Best Practice Standards as part of the Healthy Families New York system.
The home visitor's typical schedule is Monday through Friday, 8:00 am–4:30 pm, with flex time provided to cover occasional evening or weekend outreach events.
Responsibilities:
- Conduct outreach and screenings to identify eligible caregivers.
- Build trusting, long-term relationships with families through supportive, strength-based home visits throughout Jefferson County, tailored to each family's goals.
- Partner with parents — from pregnancy through early childhood — to help them access community resources and healthcare and build nurturing, stable family environments.
- Lead evidence-based parent–child activities that support healthy development and strong, loving bonds.
- Collaborate with community agencies and participate in outreach events.
Preferred Qualifications:
- Experience in home visiting and prevention services focused on the prenatal to age 5 population.
- Understanding of reflective practice, motivational techniques, and infant and child development.
- Familiarity with diverse communities and cultural sensitivity.
- Bilingual fluency in English and Spanish.
Minimum Requirements:
- Associate degree or higher in human services or a related field.
- Strong communication skills.
- Proficiency in using technology (computer, smartphone, Microsoft Office 365, Adobe PDF).
- Valid driver's license, reliable personal car, and active auto insurance policy. Work-related travel is reimbursed at current GSA rates.
- Must pass background checks, including criminal history, child abuse/neglect registry, and education and employment verification.
