Job Overview
North Central Health Care is seeking a skilled Case Manager to join our team. As a key member of our multidisciplinary team, you will provide community-based case management for individuals diagnosed with mental health and/or substance use disorders.
About the Opportunity
The Case Manager coordinates care for individuals enrolled in Comprehensive Community Services, Community Support Program, Children's Long-Term Support, or the Coordinated Services Team program by collaborating with a multi-disciplinary team. The population served includes individuals with a mental health and/or substance abuse diagnosis.
Your Key Responsibilities
* Determine ongoing eligibility for Community Treatment programs via the approved state functional screen
* Facilitate the assessment and service planning process including the identification of consumer needs, goals, objectives, strengths, barriers, interventions, and their priority in the treatment planning process
* Assess ongoing needs, including crisis situations, to determine treatment interventions and service provision
* Ensure the service plan and service delivery for each client is integrated, coordinated, and monitored
* Provide services in adherence to the pillars of Community Treatment program philosophy which include being strength-based, recovery-oriented, person-centered, flexible, trauma informed and consistent with best practice
* Complete documentation in compliance with Community Treatment standards, agency policy and applicable regulations
* Adhere to scheduled work hours with regular and predictable attendance
What We're Looking For
* Bachelor's or Master's degree in human services field
* At least one year of case management experience
* Ability to work in fast-paced environment
* Person-centered service in all interactions
* Valid driver's license and use of a personal vehicle
* Attention to detail and organized
