We are seeking an energetic, detail-oriented, and people-focused Patient Care Coordinator (PCC) to join our growing team. This is more than a front desk role—it’s an opportunity to make a real impact on patient experiences and outcomes while working alongside some of the best therapists in Colorado.
What You’ll Do
As a Patient Care Coordinator, you will:
- Provide exceptional service to every patient—greeting, scheduling, and supporting their rehab journey.
- Convert new referrals into scheduled patients and ensure treatment plans stay on track.
- Manage insurance authorizations, benefits verification, and co-pay/supply collections with accuracy.
- Coordinate daily operations with the Director of Clinical Operations and Director of Communications.
- Monitor patient care plans (POCs) and help ensure patients complete their prescribed visits.
- Track, analyze, and report key metrics to improve clinic efficiency and patient outcomes.
- Act as a communication hub between patients, therapists, and leadership.
- Contribute ideas, share priorities, and proactively flag roadblocks so the team can succeed together.

Physical Requirements
- Periods of prolonged sitting and computer work.
- Ability to move between front desk, clinic, and patient areas as needed.
