Job Title: Care Manager
Department: Care Management
Hours: Full Time, Monday through Friday 8am to 5pm. Shifts may change with patient needs, program expansion or coverage needs.
North Bend Medical Center is currently hiring a Care Manager. Located in Coos Bay Oregon along the beautiful Southern Oregon coast. North Bend Medical Center hosts over 100 providers in a variety of specialties and 400 employees.

Job Overview:
The Care Manager plays a vital role in promoting excellence in healthcare delivery for patients and their families. This position focuses on developing and overseeing health promotion initiatives, disease management strategies, care coordination, and utilization management within Primary Care teams.
Key responsibilities include:
  • Managing high-risk patients with multiple comorbidities or those at elevated risk for hospital admission or readmission.
  • Collaborating with Care Teams to create and implement individualized care plans aimed at guiding patients toward optimal health outcomes while adhering to clinical best practices.
  • Assisting patients in accessing necessary resources to support their health and well-being.
  • Supporting the Vaccine Clinic by administering immunizations to patients aged 12 and older.
  • Performing clinical procedures such as EKGs, Pulmonary Function Tests (PFTs), and managing overnight oximetry checkouts.
  • Scheduling and checking in patients for appointments to ensure smooth clinic operations.
  • Assisting the Transitions of Care team with outreach for emergency department and hospital follow-up.

The Care Manager may perform the following duties:
  • Partner with all clinical teams as a resource for care management.
  • Identify targeted patient populations that would benefit from care management services.
  • Conduct pre-visit planning to ensure care completion prior to patient appointments whenever possible.
  • Coordinate patient care services across the continuum through collaboration with patients, families, and healthcare providers to achieve optimal outcomes.
  • Communicate with patients' post-hospitalization to schedule follow-up appointments, assist with medication education, review after-visit summaries, and serve as a point of contact.
  • Collaborate with the Care Team to ensure smooth transitions of care for patients treated in facilities (inpatient or emergency department), by specialty physicians, or other healthcare providers.
  • Assist the Transitions of Care team with outreach for emergency department and hospital follow-up.
  • Engage patients in health improvement activities, provide health education, support self-management tasks, and offer counseling to patients and families.
  • Identify barriers to optimal health, problem-solve, and coordinate with the Care Team and external resources to overcome challenges.
  • Work with physician leadership to develop strategies for managing high-risk patient populations, ensuring coordinated care from office to hospital to home.
  • Develop and refine office workflows in collaboration with physicians, focusing on preventive services and chronic disease management.
  • Lead the healthcare team by example, promoting integrated nursing practices, exceptional customer service, innovation, and strong support for the physician practice.
  • Support the Vaccine Clinic by administering immunizations to patients aged 12 and older.

  • Schedule and check in patients, verifying insurance and patient information as needed.
  • Ensure safe practices and promptly report any concerns.
  • Maintain complete and accurate documentation in the medical record.
  • Collect and monitor data related to patient outcomes.
  • Perform all other duties as assigned.
Required Skills and Knowledge:
  • Able to effectively communicate with others, both written and orally.
  • Must possess critical thinking and problem-solving skills.
  • Ability to prioritize work, manage multiple demands, and demonstrate successful time management skills.
  • Excellent computer skills.
  • Ability to work both with and for the team.
  • Ability to show respect and sensitivity to cultural differences in both employees and patients.
  • Knowledge and support of organization’s goals and values.
  • Education: High school diploma.
  • Certification: CCMA or CHW certification preferable.
