This salaried position comes with a sign-on bonus up to $5,000, per aligned to one year of commitment! Eligible for every other Friday off after successfully completing six months of employment and meeting productivity goals.
We are seeking a full-time, salaried Maternity Care Coordinator to promote and protect the health and well-being of the Medicaid Eligible Individuals of the Care Program in Madison County. The schedule is Monday-Friday (8am-5pm). Candidate must have minimum of a Licensed Baccalaureate Social Worker (LBSW) from an accredited college. Candidate must also have a current Alabama Driver’s License and Auto Insurance (State minimum requirements).
Qualifications:
- Two (2) years of health care experience in a perinatal setting preferred
- Excellent Computer/Communication Skills
- Minimum of a Licensed Baccalaureate Social Worker (LBSW) from an accredited college
- Pleasant and cooperative attitude
Essential Functions
- Assist Eligible Individuals (EIs) and DHCP with navigation of Maternity healthcare systems.
- Develop a team approach Care Plan including the pregnant client and the DHCP and other community resources and agencies.
- Complete face to face visits with clients to build rapport and identify services or referrals needed to work towards goal attainment.
- Ensure the pregnant eligible individual has access to covered services, proper referral, and resources for the Maternity Care program.
- Willingness to travel and/or conduct home visit as required for the job.
Salary : $45,000
