RN Case Manager Job Description
Shift - Monday – Friday 8am -4:30 pm with weekend rotation
Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, conducting assessments, and ensuring that patients receive the necessary services throughout their treatment journey. You will work closely with patients, families, and healthcare professionals to develop individualized care plans that address medical, emotional, and social needs.
Responsibilities
- Conduct comprehensive patient assessments to determine healthcare needs and develop appropriate care plans.
- Monitor vital signs and evaluate patient conditions to ensure optimal health outcomes.
- Collaborate with interdisciplinary teams to coordinate inpatient care and facilitate transitions between different levels of care.
- Provide education and support to patients and families regarding treatment options, medications, and self-care strategies.
- Advocate for patients within the healthcare system to ensure they receive the necessary services.
- Document all patient interactions and updates in accordance with regulatory standards.
- Participate in emergency medicine situations as needed to provide immediate care.
- Maintain up-to-date knowledge of best practices in nursing.
Qualifications
- Current Registered Nurse (RN) license in the state of practice
- Current BLS certification
- At least 1 year of experience in case management required
- Strong skills in patient assessment and service delivery.
- Ability to work collaboratively within a team environment while also demonstrating independent decision-making skills.
- Excellent communication skills to effectively interact with patients, families, and healthcare professionals.
