Duties
- Deliver direct patient care by assessing residents’ health status, administering medications, and performing routine physical examinations with precision and compassion
- Develop and implement individualized care plans in collaboration with interdisciplinary teams, ensuring adherence to best practices and regulatory standards.
- Monitor vital signs, manage acute pain, and respond swiftly to emergent situations using critical care techniques including airway management and ventilator support
- Conduct comprehensive patient assessments utilizing advanced EMR/EHR systems to document medical records accurately and efficiently
- Perform specialized procedures while maintaining aseptic technique
- Coordinate discharge planning, case management activities, and family education to support ongoing resident well-being and transition of care
Requirements
- Valid RN license in the state of IL.
- Current BLS certification
- At least 2 years of RN experience
- At least 1 year of LTC / SNF experience.

Pay: $57.00 per hour
