Nature of the Work

The Navigator provides holistic and comprehensive case management and support services to clients eligible for assistance through the McKinney-Vento Program. The Navigator is responsible for providing services that assist clients in developing independent and sustainable lifestyles. The Navigator, with collaborative partners as well as other staff, forms an interdisciplinary team that develops treatment goals and plans. The Navigator monitors client progress and ensures the clinically appropriate services to comply with policies and agency requirements. The Navigator is strategically and tactically important and will directly affect AOL’s mission accomplishment and clients’ quality of life.

Essential Duties

The Navigator’s essential duties are listed below. Additional duties may be assigned when necessary, and Avenue of Life expects the incumbent to exercise initiative to anticipate and prevent problems.

• Provide holistic and comprehensive case management services to Impact Model clients, including intake assessment (Arizona Self-Sufficiency Matrix), benefits assessment, goal setting, long-term case plan development, weekly case plan development, progress monitoring, individual money management education, tenant housing education, employment education, advocacy, and referrals, and reporting and compliance.
• Learn and implement case management best practices.
• Provide intensive strengths-based case management services to a minimum caseload of forty to fifty active families.
• Provide emergency assistance to walk-in clients, including resources (food, clothing, hygiene products, etc.) and referrals from community partners.
• Update client files daily to record continuity of care and maintain complete confidentiality of all client records.
• Teach program classes as assigned.
• Complete data entry into online systems (CCL, MAACLINK, SureImpact, etc.) and remain compliant with grants and data tracking goals.
• Maintain criteria and reporting requirements on grants specific to housing and utilities.
• Participate in supervision meetings with the Program Director as required.

Qualifications and Compensation:

• Preferred LMSW with Missouri Licensure
• Must be bilingual (English and Spanish)
• Salary ($60,000 - $65,000 annually commensurate with experience)
