Overview:
Join a mission-driven organization supporting a Medicaid-funded social care initiative to improve health equity. This is a non-clinical, contract-to-hire role helping Medicaid members access vital community resources and services.
What You’ll Do:
· Screen Medicaid members for social needs
· Connect individuals to community & social services
· Perform outreach by phone, virtual, and in-person
· Track referrals and follow-ups in a care coordination system
· Document services and meet productivity goals
What We’re Looking For:
· HS diploma/GED required
· 2–3 years in community health, social services, outreach, or care navigation
· Medicaid or underserved population experience preferred
· Bilingual a plus
· Strong communication & organization skills
· Driver’s license & reliable transportation preferred
