Position: Public Health Nurse II
Full Time: Monday through Friday/ 8:00 a.m.-4:00 p.m.

Position Overview:
The Public Health Nurse II is responsible for managing the Noble County Health Department (NCHD) Vaccination Clinic including appointments, vaccinations, and outreach throughout the lifespan of all and across diverse communities, lead case management per Indiana Department of Health (IDOH) guidelines, and assisting in managing communicable disease including surveillance NBS reporting and outreach per IDOH guidelines.
Essential Duties:
● Manage the care of clients using established protocols.
● Ensure compliance with all applicable County Health Department policies and procedures, Indiana Department of Health (IDOH) standards, HIPAA regulations, and all other federal and state laws relating to privacy, confidentiality and service delivery.
● Administer immunizations to all age groups.
● Monitor clients prior to and post vaccination.
● Provide education to families about vaccinations.
● Screen for and provide referrals to area agencies for families receiving vaccines that may need additional services.
● Plan, organize and implement off-site clinics.
● Conduct after-hours shot clinics as needed.
● Complete necessary IDOH paperwork, documentation, reporting, and requests, such as off-site clinic requests and borrowing reports.
● Remain up-to-date on knowledge of vaccination types, schedules, storage and administration through continuing education and staff meetings.
● Organize and conduct school vaccine clinics in partnership with county schools and school nurses.
● Advise parents, students, and schools on needed routine vaccinations for various age groups based on CDC guidelines.
● Organize off-site school vaccine clinics with virtual and/or paper sign-ups (whichever is available).
● Respond to telephone calls and emailed questions from school nurses, including scheduling appointments, verifying vaccine records, and verifying student appointments surrounding vaccinations and lead.
● Perform TB testing.
● Offer lead testing to all children age 1-5 years, collect blood samples with parental consent.
● Conduct case management for children with blood lead levels 3.5 ug/dl or above including but not limited to phone interviews, provider referrals, and home visits.
● Provide lead education to parents. Maintain lead education resources in English, Spanish, and Arabic.
● Maintain health records according to IDOH and Indiana State statutes.
● Provide accessible, equitable clinical services, such as those related to communicable diseases, to meet the demands of the community.
● Assist in investigating reportable communicable diseases, provide education to infected individuals and their close contacts, submit appropriate specimens to lab, complete/file epidemiological reports and submit to IDOH as required. Provide case management as needed.
● Assist in initiating a public health investigation within 24 hours for immediately reportable conditions reported.
● Assist in initiating a public health investigation within two business days for non-immediately reportable conditions reported.
● Prepare and administer medications and or treatments as prescribed by physicians.
● Respond to telephone calls and questions from members of the public, including scheduling appointments, making referrals, counseling, and providing health information as appropriate.
● Establish partnerships for housing, food security, and interpretation services to assist in case management services for patients with tuberculosis and latent tuberculosis (LTBI) in the community.
● Attend trainings and obtain certifications and/or completion certificates for sessions or courses recommended or required by the county or IDOH.
● Engage with the community in a professional manner through all communication methods: in-person, telephone, email, social media, etc. Answer questions and refer to the appropriate person or department as needed.
● Respond to public health emergencies on a 24-hour basis.
● Perform related duties as assigned.
Requirements:
● High School Diploma or GED equivalent PLUS Nursing degree or degree in similar field
● Experience in public health field preferred
● Ongoing continuing education
● Up to date Healthcare provider CPR certification
● TB Certification (may be obtained upon hire with Health Department)
● Accessible by phone at all times in case of emergency
● Thorough knowledge of and ability to practically apply federal, state, and local rules and regulations governing public health with the ability to conduct investigations, perform testing and sampling, and assure compliance with all legal requirements
● Work independently and with other employees
● Follow the policies and procedures of the NCHD, Noble County government, and IDOH
● Comply with federal, state, and local rules and regulations regarding public health
● Prioritize and manage time to accomplish duties and assignments within the work day
● Maintain working relationships with contractors, engineers, city and town officials, law enforcement, state and federal representatives, and co-workers
● Must be able to sit, stand, walk, keyboard, lift and carry objects 25 pounds and under, crouch, kneel, use close vision, and clearly communicate
● May be exposed to physical and health hazards associated with field work including but not limited to construction sites, food establishments, sewage, chemicals, animal/insect bites, infectious diseases, sharps hazards, and inclement weather
● Up to date on all ACIP recommended vaccines.
