Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, and temperature
- Give medication reminders following care plans and HIPAA guidelines
- Provide meal preparation and nutritional support tailored to individual dietary needs
- Support patients with memory care, dementia, Alzheimer's, or other cognitive impairments
- Use assistive devices such as Hoyer lifts or resident lifts to ensure safe transfers and mobility
- Observe and report changes in patient condition or behavior to care team
- Perform light housekeeping
- Follow prescribed care plans and document services provided accurately
- Support patients during social activities and promote engagement for overall well-being
- Ensure compliance with all safety protocols including CPR, first aid, and infection control procedures
- Proven experience in home & community care, assisted living, nursing homes, or hospice care
- Knowledge of patient observation techniques and vital signs monitoring
- Ability to assist with ADLs and provide compassionate patient care
- Familiarity with medication reminders and care planning processes
- Experience working with people with dementia, arthritis, or mobility challenges.
- Strong communication skills for effective interaction with clients, families, and healthcare teams
- Ability to perform heavy lifting safely when required (e.g., resident transfers)
- CPR and First Aid certification preferred; ongoing training provided if necessary
- Knowledge of HIPAA regulations to ensure patient confidentiality and privacy
- Experience working with diverse populations including seniors, individuals with disabilities, or those requiring memory care support
