Responsibilities
- Provide direct patient care and assist with daily living activities
- Help with medication administration as directed
- Monitor vital signs regularly and report changes
- Support personal hygiene and grooming needs
- Assist patients with mobility and transfers
- Maintain accurate records of care and patient progress
- Communicate effectively with patients, families, and healthcare team
- Respond promptly to emergencies as needed
- Operate and maintain home health care equipment safely
- Provide transportation assistance when required
Preferred Qualifications
- 3 years of experience in home health care
