NEEDED: Pharmacists

LOCATIONS:

• Salt Lake City, UT 84116
• Gunnison, UT 84634

SCHEDULE: As Needed

QUALIFICATIONS:

• Must maintain all licenses required to lawfully perform the Pharmacist services and ensure they remain current.
• Maintain current, active, and unrestricted licenses in the State of Utah applicable to pharmacists
• Must submit proof of board certification or board eligibility in the Pharmacia field.
• Maintain a valid Controlled Substance License and Drug Enforcement Administration certification with no disciplinary actions.

JOB DUTIES:

• Prepare medications according to prescriptions written by authorized medical practitioners;
• Provide drug information and consultation to physicians, nurses, patients, and other clinical staff;
• Order and maintain a secure inventory of drugs, chemicals, and controlled substances;
• Fill and refill prescriptions as authorized;
• Prepare reports on medication and drug usage patterns; and
• Perform other pharmaceutical tasks assigned by DHHS staff.
