Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Perform patient observation to detect changes in condition and communicate findings promptly to healthcare teams.
- Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring the overall safety of the home setting.
- Support patients’ emotional well-being through compassionate communication and engaging activities tailored to their needs.
- Work collaboratively with other caregivers and healthcare professionals to coordinate comprehensive home & community care services.
