- Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records in compliance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Support medication administration following prescribed schedules and safety protocols
- Maintain a clean and safe environment through regular cleaning, sanitation, and organization of living spaces
- Use specialized equipment such as Hoyer lifts or transfer devices to ensure safe mobility and prevent injury during transfers
- Document care plans, patient progress, and incident reports accurately to ensure seamless communication among team members
- Proven experience in home health, assisted living, nursing home care, or working with individuals with developmental disabilities or dementia is highly preferred
- Certification in First Aid and CPR is required; additional certifications such as HIPAA training or hospice care are advantageous
- Ability to lift heavy objects safely and perform physically demanding tasks as needed
- Strong observational skills for patient monitoring and recognizing changes in condition or behavior
- Compassionate attitude with excellent communication skills to engage effectively with residents, families, and team members
- Knowledge of care plans, medication administration procedures, and safety protocols for vulnerable populations
- Experience working with individuals with autism or memory care needs is a plus
