Position Summary
The Social Services Director is responsible for overseeing the facility's social services program and ensuring residents receive the psychosocial support necessary to maintain the highest practicable level of well-being. This role works closely with nursing, therapy, admissions, and interdisciplinary team members to support resident care, discharge planning, and family communication.
Key Responsibilities
Coordinate and oversee social services for residents in the skilled nursing facility
Conduct psychosocial assessments and develop individualized care plans
Support residents and families through admissions, transitions, and discharge planning
Advocate for resident rights and quality of life
Provide counseling, support, and community resource referrals
Facilitate family meetings and interdisciplinary care conferences
Assist with discharge planning and safe community transitions
Maintain compliance with state and federal regulations
Collaborate with facility leadership to support resident satisfaction and quality outcomes
Serve as a compassionate resource for residents, families, and staff
Qualifications
LSW preferred
LLSW strongly preferred
Experienced non-licensed social workers with skilled nursing or long-term care experience will also be considered
Previous experience in a skilled nursing facility, long-term care, hospital, or healthcare setting preferred
Strong knowledge of discharge planning, care conferences, and resident advocacy
Excellent communication and relationship-building skills
Ability to manage multiple priorities in a fast-paced environment
Compassionate, organized, and resident-focused approach
