Job Description: Centralized Admissions RNLocation: Preferred candidate resides in New York City area Coverage: Multiple Skilled Nursing Facilities across 3 states Position Type: Full-Time, Remote Hybrid (minimal travel required during the year) Compensation: $100,000 – $150,000 annuallyPosition SummaryThe Centralized Admissions RN will serve as the primary clinical liaison for admissions across multiple skilled nursing facilities in three states. This role requires a registered nurse with strong analytical skills to evaluate clinical data, ensure appropriate placement, and streamline the admissions process. The ideal candidate will be highly organized, detail-oriented, and experienced with PCC (PointClickCare) systems. This is a remote hybrid position with only minimal travel required throughout the year.Key ResponsibilitiesAdmissions CoordinationManage centralized admissions for multiple facilities, ensuring timely and accurate placement decisions.Review and analyze clinical data to determine patient eligibility and appropriate level of care.Collaborate with facility administrators, nursing staff, and referral sources to facilitate smooth transitions.Clinical Data AnalysisCollect, interpret, and document patient medical information to support admissions decisions.Ensure compliance with state and federal regulations regarding patient admissions.Provide clinical insight to leadership regarding trends and opportunities for improvement.System ManagementEnter and maintain accurate patient information in PCC (PointClickCare).Generate reports and track admissions metrics across facilities.Train and support staff in PCC usage as needed.Communication & CollaborationAct as the central point of contact for referral sources, hospitals, and families.Coordinate with interdisciplinary teams to ensure continuity of care.Provide regular updates to leadership on admissions activity and outcomes.QualificationsEducation: Registered Nurse (RN) license required.Experience: Minimum 3–5 years of nursing experience, preferably in skilled nursing or long-term care admissions.Technical Skills: Proficiency with PCC (PointClickCare) and strong data analysis skills.Other Requirements:Excellent communication and organizational skills.Ability to manage admissions across multiple facilities and states.Preference for candidates residing in the NYC area.Comfortable working in a remote hybrid environment with occasional travel and visits to our central office in Brooklyn required.Compensation & BenefitsSalary range: $100,000 – $150,000 annually, based on experience.Comprehensive benefits package including health, dental, vision, 401(k), paid time off, and professional development opportunities.
