- Provide social services to patients and their families at OU Health under general supervision.
- Conduct discharge planning evaluations and psychosocial assessments.
- Counsel patients and others regarding diseases, treatments, social or financial situations, emotional stress, and other problems.
- Provide crisis counseling and intervention for high social risk cases including child abuse or neglect and physical abuse.
- Act as liaison between patients/families and hospital staff and other agencies.
- Evaluate effectiveness of services and adjust patient care as needed.
- Assist in discharge planning and referrals to community services and post-acute care facilities.
- Assist patients and families with adverse reactions or difficult adjustments to illness, treatment, or changes in functional status.
- Utilize specialized interviewing and communication skills with children.
- Advocate for patients on issues related to informed decision making, end-of-life care, goal setting, and ethical dilemmas.
- May prepare statistics concerning referrals and extended stays.
- May supervise students or newly hired employees.
- Job Requirements
- Hospital experience preferred.
- Knowledge of principles and practices of social and psychological casework.
- Knowledge of social and welfare resources available in the community and hospital.
- Analytical skills to interpret situations and determine appropriate actions.
- Ability to explain technical data understandably to patients, families, and hospital staff.
- Good interpersonal skills.
- Master’s Degree in Social Work from an accredited school.
- LMSW (Licensed Master Social Worker) strongly preferred.
