Registered Nurse (RN) – Home Health
Location: Stamford, CT / Norwalk, CT
Paid Per Visit – Starting at $55 per visit

Your Role
• Conduct patient admissions and develop individualized Plans of Care
• Obtain medical histories and perform physical assessments
• Monitor patient status, vital signs, and clinical progress in the home setting
• Deliver skilled nursing care in compliance with HIPAA and agency standards
• Evaluate patient outcomes and adjust care plans as needed
• Collaborate with the interdisciplinary team to ensure comprehensive care
• Coordinate services with LPNs, HHAs, and therapy providers
• Complete accurate documentation and submit clinical notes in a timely manner
Qualifications
• Current Registered Nurse (RN) license in Connecticut
• OASIS and WellSky (Kinnser) experience preferred
• Medical-surgical and wound care experience preferred
• Physical exam within the last 12 months
• TB test within 12 months and current CPR certification
• Valid driver’s license and reliable transportation
• Ability to attend a 1–3 day orientation at our Stamford office
