Overview:
We are seeking compassionate and skilled nurses to join our Nightingale Care team, delivering high-quality, one-on-one medical care in the comfort of our patients’ homes. As a Home Health Nurse, you will perform start-of-care visits, complete comprehensive assessments as needed, and manage patient discharges, ensuring continuity of care from admission through transition.
In this role, you will assess patient needs, develop and implement personalized care plans, administer medications and treatments, and provide education to patients and their families on disease management and wellness strategies. You will build strong, trusting relationships while collaborating closely with physicians, therapists, and other healthcare professionals to promote optimal outcomes.

Responsibilities:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status (OASIS)
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans as needed
- Educate patients and their families on managing their health conditions
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date medical records
Qualifications:
- Valid Registered Nurse (RN) license in the state of Ohio
- Minimum of 1 years of nursing experience, preferably in home health
- Proficient in medical record systems and documentation
- Knowledge of aseptic technique and infection control practices
- Strong patient assessment skills
- Experience working with individuals with developmental disabilities is a plus
- Ability to work independently and prioritize tasks effectively
- Excellent communication and interpersonal skills
