MDS Coordinator

Position Summary
The MDS Coordinator at Nightingale at Stonegate is responsible for coordinating and completing the Minimum Data Set (MDS) in accordance with CMS, Medicare, Medicaid, and Arkansas state regulations. This role plays a critical part in ensuring accurate clinical assessments, appropriate reimbursement, and the development of individualized resident care plans. The MDS Coordinator works collaboratively with the interdisciplinary team to support quality outcomes and regulatory compliance.
Essential Duties and Responsibilities
- Coordinate and complete MDS assessments in compliance with CMS guidelines and facility policies
- Ensure timely completion and submission of OBRA, PPS, IPA, SCSA, and discharge assessments
- Accurately schedule and manage the MDS Assessment Reference Dates (ARDs)
- Review medical records and clinical documentation to ensure accuracy and completeness
- Participate in care plan development and interdisciplinary team meetings
- Monitor and ensure compliance with Medicare reimbursement regulations and RUG/PDPM rules
- Educate nursing and ancillary staff on proper documentation practices related to MDS
- Conduct audits to identify documentation gaps and implement corrective actions
- Serve as a resource during state surveys, audits, and quality assurance reviews
- Communicate effectively with billing, therapy, nursing, and administrative staff
- Maintain strict confidentiality and compliance with HIPAA regulations
- Assist with Quality Measures (QMs) and Quality Reporting Program (QRP) initiatives
- Perform other duties as assigned by the Director of Nursing or Administrator
Required Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with an active Arkansas license
- Minimum 1–2 years of MDS coordination experience in a skilled nursing facility preferred
- Working knowledge of CMS regulations, MDS 3.0, and PDPM
- Strong clinical assessment and critical thinking skills
- Proficiency in electronic health records (EHR) and MDS software systems
- Excellent organization, time management, and attention to detail
- Strong written and verbal communication skills
Preferred Qualifications
- RACCT or AANAC certification (or willingness to obtain)
- Previous survey experience and knowledge of Arkansas longterm care regulations
- Experience working collaboratively with therapy and case management teams
