**Responsibilities:**
* Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
* Develop and implement care plans in collaboration with the healthcare team
* Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care
* Participate in Shift Reports and handovers to maintain continuity of care
* Collaborate with Nursing Assistants and other caregivers to ensure a safe and supportive environment
* Monitor and report any changes in patient condition or concerns to the Nursing Supervisor
* Maintain accurate and up-to-date patient records and reports
**Requirements:**
* Current LPN licensure in the State of Arkansas
* Minimum 1 year of experience in a long-term care setting
* Strong communication and interpersonal skills
* Ability to work effectively in a fast-paced environment
* Basic computer skills and experience with electronic medical records
