This role involves assisting with daily living tasks such as bathing, meal prep, dressing, light housekeeping, companionship, and offering support or reminders to help clients maintain their independence. Position summary:
Deliver reliable, community living support to adults in New Heights residential settings promoting independence, safety, skill development, and community inclusion, in alignment with each person’s IPOS within a 24/7 staffing model.
Key responsibilities:
• Implement each person’s IPOS and any safety/behavior plans; use least-restrictive, trauma-informed approaches
• Support daily living: meals, light cleaning, dressing, hygiene, and safe mobility (as authorized)
• Provide medication reminders per policy (no med administration unless specifically authorized/trained)
• Support community participation: errands, appointments, recreation, social activities
• Document services, progress, and incidents accurately and on time in the approved system (e.g., EVV/HHAeXchange)
• Communicate timely shift updates and observations to the Administrator/Program Director; collaborate with the care team
• Protect privacy and follow Recipient Rights, HIPAA, infection control, and all HCBS/CMS, payer, and agency policies
Qualifications
• High school diploma or GED; at least 18 years old
• Six months of residential/community direct support experience preferred
• Reliable, compassionate, and able to work a consistent schedule (days/evenings/overnights/weekends as assigned)
• Valid Michigan driver’s license and reliable transportation; auto insurance if driving individuals
• Able to pass background checks and complete required trainings (Recipient Rights within 30 days, CPR/First Aid, etc.)
• Professionalism, clear communication, and basic documentation skills
Work environment
• Services occur in apartment-style residential homes and community locations across Oakland County; travel between sites may be required
• Schedules are determined by authorized IPOS hours and staffing needs; assignments may include rotating or fixed 8- or 12-hour shifts
• Role involves light to moderate physical activity (household tasks, community walking) and safe assistance with mobility as authorized
Compensation and schedule
• $17.00 per hour
• Full-time schedule aligned to authorized IPOS hours and program coverage needs
