Responsibilities
- Conduct comprehensive physical examinations, diagnose and treat acute and chronic illnesses in a geriatric population.
- Develop and implement individualized care plans that address residents' medical, physical, and psychological needs.
- Prescribe medications, order and interpret diagnostic tests, and perform procedures within your scope of practice.
- Collaborate with physicians, nurses, social workers, and therapists to ensure coordinated care.
- Educate residents and families about health conditions and treatment options.
- Advocate for residents' well-being and ensure they receive appropriate care.
- Document patient care accurately and thoroughly in the electronic health record (EHR).
- Active certification as a Nurse Practitioner (NP) with a focus on geriatrics.
- Minimum 2 years of clinical experience in a geriatric setting (nursing home, long-term care, or geriatric clinic).
- Strong clinical skills in geriatric assessment, diagnosis, and treatment.
- Excellent communication, collaboration, and interpersonal skills.

- Proficient in EHR documentation.
