The LPN provides high-quality nursing care to residents while working closely with Registered Nurses, physicians, and the interdisciplinary team to promote each resident's health, comfort, and well-being in our long-term care and rehabilitation facility.
Qualifications
- Current Licensed Practical Nurse (LPN) license
- Current BLS/CPR certification required
- Strong clinical and assessment skills
- Experience in long-term care or rehabilitation preferred, but not required
- Excellent communication and interpersonal skills
- Compassionate, dependable, and resident-focused
- Ability to work effectively in a team environment
- Provide direct nursing care to residents in accordance with individualized care plans.
- Administer medications and treatments as prescribed.
- Monitor residents' conditions and promptly report changes to the RN or physician.
- Accurately document assessments, treatments, and resident responses.
- Assist with admissions, discharges, and physician orders.
- Collaborate with the interdisciplinary team to provide exceptional resident care.
- Supervise Certified Nursing Assistants (CNAs) and support quality resident care.
- Maintain compliance with all state, federal, and facility policies and regulations.
- Promote a safe, clean, and compassionate environment for residents and families.
