Role Description This full-time, on-site role for Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) is located at Oak Knoll Health and Rehabilitation in Birmingham, AL. Nursing team members provide direct patient care, including monitoring vital signs, administering medications and treatments, and supporting daily living activities in coordination with interdisciplinary care plans. They document assessments and interventions accurately, communicate changes in patient condition to providers and families, and follow facility protocols for safety and infection control. LPNs and RNs collaborate with other healthcare professionals to plan and evaluate care, participate in care conferences, and support patient and resident education. The role also involves maintaining professional standards, adhering to regulatory requirements, and contributing to a positive, respectful care environment.

Qualifications

• Active LPN or RN license in the state of Alabama, in good standing, with eligibility to practice in a long-term or post-acute care setting.
• Competence in patient assessment, medication administration, wound care, and monitoring of vital signs and symptoms.
• Strong documentation skills, including accurate charting in electronic or paper medical records and adherence to facility policies.
• Effective communication and interpersonal skills for working with patients, families, and interdisciplinary care teams.
• Knowledge of infection control practices, safety procedures, and regulatory standards applicable to long-term and rehabilitative care.
• Ability to prioritize tasks, manage time effectively, and respond calmly and professionally to urgent or emergent situations.
• Demonstrated commitment to compassionate, person-centered care and respect for individuals of diverse backgrounds.
• Previous experience in skilled nursing, long-term care, or similar healthcare settings preferred but not required.
• Basic computer skills for accessing and updating patient records; familiarity with EHR systems is a plus.
• CPR certification and other required clinical credentials, or willingness to obtain them upon hire.
