Position: Clinical Admissions Coordinator – Full Time
We are seeking a clinically experienced professional to join our team as a Clinical Admissions Coordinator, playing a vital role in evaluating patient needs, guiding care transitions, and supporting appropriate admissions into our center.
This position is ideal for a nurse or clinically trained individual looking to step into a more collaborative, non-bedside role while continuing to make a meaningful impact on patient outcomes.
Qualifications
- Licensed Nurse (LPN or RN) preferred, or equivalent clinical experience
- Bachelor’s degree preferred from an accredited college or university
- Strong understanding of medical case management and care transitions
- Knowledge of geriatric patient populations and post-acute care settings
- Working knowledge of funding resources including Medicare (A, B, C, D), Medicaid, Managed Care, and private insurance
- Insurance authorization experience preferred
- Ability to interpret clinical information and medical terminology
- Strong communication and relationship-building skills
- Marketing or referral development experience is a plus
Key Responsibilities
- Review and coordinate incoming referrals, using clinical judgment to assess appropriateness for admission
- Serve as a liaison between hospitals, providers, patients, families, and internal care teams
- Conduct thorough needs assessments and help guide patients and families through the admissions process
- Collaborate with interdisciplinary teams to support safe and appropriate care transitions
- Communicate funding options and assist with navigating insurance and authorization processes
- Identify and proactively address barriers to admission
- Provide support and advocacy for patients and families during times of transition, illness, or crisis
- Represent the center professionally with referral sources and community partners
