We are currently looking for passionate and motivated individual to join our team in a leadership role. This role focuses on helping adults with Autism Spectrum Disorder (ASD) foster independence and develop essential independent living skills. We are seeking candidates dedicated to making a significant impact in the lives of our clients.
Program Overview:
Nexus launched this new program in 2021, serving individuals with ASD who live in their own apartments, alone or with a roommate, under a case management model. We currently support five individuals in Woburn and five in Lowell.
Position Details:
This is a full-time (40 hours) Lead Case Management position with benefits.
- Salary: $26.54 per hour.
- Supervision: You will work directly under the Program Director, who provides ongoing support sessions and training opportunities for professional growth.
Responsibilities include:
- Monitoring attainment of Life Plan goals.
- Monitoring health, wellness, medical appointments, and medication compliance.
- Promoting independent living skills and the development of social, recreational, and community opportunities.
- Maintaining compliance with all DDS and Nexus regulations, policies, and directives.
Schedule: The schedule is Monday, Wednesday and Friday 9am-5pm and Tuesday, Thursday 11am-7pm with some flexibility on an as needed basis.
Requirements:
- A current, valid driver's license and available vehicle are required.
- Willingness to travel to Woburn and Lowell sites.
- Two years of experience in the field or related course of study is required. A bachelor's degree is preferred.
- CPR/First Aid certification is mandatory.
