The Opportunity
As a Patient Care Assistant, you will provide hands-on care and behavioral support as part of an interdisciplinary team. You will help patients with daily activities, share important observations with nursing staff, and contribute to a safe, structured, and compassionate environment.
What You’ll Do
- Assist patients with bathing, dressing, grooming, feeding, turning, positioning, mobility, and other activities of daily living.
- Obtain and document vital signs; promptly report changes in a patient’s physical condition, behavior, or mental status.
- Perform point-of-care testing and specimen collection within your training and facility procedures.
- Complete assigned 15-minute observation checks and support one-to-one patient monitoring.
- Participate in bedside shift reports and share observations with the care team.
- Engage patients in therapeutic activities and group programming.
- Use approved de-escalation and crisis prevention techniques to support patient and staff safety.
- Assist licensed nursing staff with wound care support and minor clinical procedures, as directed.
- Maintain accurate documentation and follow infection prevention, privacy, and patient safety procedures.
- Help orient and support newer team members as needed.
- High school diploma or equivalent required.
- At least one year of direct patient care experience in a hospital, behavioral health, rehabilitation, or long-term care setting required.
- Experience supporting patients with behavioral health conditions, cognitive impairments, or psychiatric disorders.
- Strong communication skills and the ability to provide compassionate care while maintaining professional boundaries.
- Ability to use an electronic health record and document care accurately.
- BLS certification from the American Heart Association or American Red Cross required, valid for at least six months from the date of hire.
- Ability to complete required de-escalation training within 30 days of hire.
