Pulmonary and Acute Care Nurse Practitioner
Location: Senior Nursing Home Facility/Long Term Care Rehab Center
Job SummaryWe are seeking an experienced APRN in pulmonary and acute care to deliver high-quality, patient-centered care in skilled nursing and long-term rehabilitation settings. This in-person role focuses on managing complex respiratory and acute conditions, including higher-acuity patients, and requires availability 5 days per week, with occasional after-hours on-call coverage.
ResponsibilitiesProvide pulmonary and acute care services in skilled nursing, rehabilitation, long-term care, and post-acute settings
Admit new patients and complete comprehensive pulmonary and acute care admission evaluations
Manage and complete patient discharge evaluations, including respiratory stability assessment and care coordination
Perform comprehensive and focused respiratory assessments and physical examinations
Evaluate, diagnose, and manage acute and chronic pulmonary conditions (e.g., COPD, asthma, pneumonia, respiratory failure)
Manage noninvasive ventilation therapies, including CPAP and BiPAP initiation, adjustment, and monitoring
Manage patients with tracheostomies, including routine trach care, assessments, and coordination of care
Collaborate on ventilator and tracheostomy weaning plans in coordination with physicians and respiratory therapy
Prescribe and manage respiratory and acute care medications in accordance with state regulations and clinical best practices
Manage acute care conditions in post-acute settings, including early identification and treatment of clinical deterioration
Implement timely interventions to prevent unnecessary hospitalizations and Return-to-Hospital (RTH) events
Collaborate with interdisciplinary teams to manage acute changes in condition and escalate care appropriately
Participate in initiatives focused on reducing hospital admissions through proactive pulmonary and acute care management
Interpret and trend respiratory diagnostics, including ABGs, imaging, labs, pulse oximetry, and ventilatory data (as applicable)
Monitor patient response to treatments and intervene promptly to prevent respiratory decompensation
Coordinate care with pulmonologists, attending physicians, respiratory therapists, nursing staff, and facility leadership
Participate in pulmonary rounds and interdisciplinary care planning
Complete accurate and timely documentation in the EMR/EHR
Educate patients, families, and staff on respiratory conditions, CPAP/BiPAP use, tracheostomy care, and escalation protocols
Participate in initiatives to reduce respiratory-related hospital readmissions and emergency transfers
Ensure compliance with CMS, state regulations, and facility respiratory care and infection control policies
Active State APRN license
Active NPI number
Minimum of 3 years of nursing experience (tracheostomy care, skilled nursing or post-acute care preferred)
Current BLS and ACLS certification
Strong clinical assessment, communication, and independent decision-making skills
Experience with EMR/EHR systems (PointClickCare and/or Matrix preferred)
Ability to work independently in a fast-paced clinical environment
Knowledge of post-acute care and geriatric best practices
Working knowledge of state and federal healthcare regulations, HIPAA, and patient safety standards
Excellent written and verbal communication skills for documentation and interdisciplinary collaboration
