PRP Direct Care Caseworker

Summary:  A currently competent Psychiatric Rehabilitation Direct Care Staff delivers care to adolescent & adult outpatients from the ages 5 to 65 years who due to the severity of their mental illness are experiencing significant deficits in there independent living skills. The Direct Care Staff will utilize patient management skills necessary to provide a safe and secure therapeutic environment, to conduct task orientated and didactic groups, to perform therapeutic interventions, and to provide one to one support for patients as necessary. The Direct Care Staff works in coordination with a Psychiatric Rehabilitation Specialist.

Essential Duties & Responsibilities:               

• 
  Responsible for assisting with patient management, including safety and security.
• 
  Responsible for developing and assuring that rehabilitation services meet the needs of the individuals served by the program.
• 
  Responsible for completing the safety risk assessment within 10 working days, benefits status assessment within 30 calendar days and rehab assessment within 30 calendar days.
• 
  Responsible for completing Individual Rehabilitation Plan (IRP) and continuing care plan within 30 calendar days.
• 
  Responsible for completing monthly summaries on or before end of each month.
• 
  Responsible for completing IRP reviews every 3 month (child & adolescent) or 6 months (adults) on or before end of the month when they are due
• 
  Responsible to conduct both onsite and offsite groups.
• 
  Responsible for facilitating educational and task oriented groups.
• 
  Responsible for implementing the rehabilitation activities outlined in the individual's Rehabilitation Plans.
• 
  Responsible to assure that patients are able to follow their scheduled activities.
• 
  Responsible for documentation pertaining to treatment provided.
• 
  Responsible for assessing patient strength, skills, and needs in the following areas independent living, housing, employment, self-administration and management of medications; health status and leisure activities; mobility and transportation; family and social relationships, educational and vocational training; adaptive equipment needs and resources.
• 
  All clinical work reviewed and monitored weekly/monthly/quarterly to ensure that 100% of the patient’s records are completed through chart audits.
• 
  Responsible for meeting all benchmarks set by the organization
• 
  Responsible for attending all staff meetings and scheduled clinical meeting/reviews.
• 
  Responsible for being available for emergency therapy sessions with patients while on duty.
